2011 PAYMENT POLICIES CONTRACT
FREE WEEK: You are
entitled to 1 free week per contract year. That
week you are not responsible for child care fees. You must give a 2 weeks
written notice that you will be using your free week.
DISCOUNTS: A
Discount of 5% is offered to the second child when a first child is
enrolled full time.
REFERRAL DISCOUNT:
5% Discount is offered for every enrollment referral of equal or greater time
for as long as referral is enrolled. Limit of one referral discount per
family.
TUITION FEES:
TUITION FEES are payable on or before Friday of the week
prior to service. Fees are due every week in which the child is
enrolled in the Center. The weekly fee applies to weeks with Holidays,
Snow Days, Sick Days and/or days you choose not to send your child to the
Center.

Full Time rate, 25
hours and over: $236; Part Time
rate, 24 hours and under: $210; Day
rate, $74.00 per day up to 8.5 hours
18 months- 35
months:
Four or Five full
days/25 + hrs/week - $184.00 per week; Three full days/25 - hrs/week- $158.00 per week; Two or Three full
days - $53.00 per day (2 day minimum);
Preschool (8.30-11.30) - $37.00 per day.
36 months - 6
years:
Four or Five full
days/25 + hrs/week - $158.00 per week; Three full days/25 - hrs/week- $131.00 per week; Two or Three full
days - $47.00 per day (2 day minimum); Preschool (8.30-11.30) - $26.00
per day.
School Aged:
Before and After
care, Five days per week: $79.00
; Before and Aftercare Daily, 2 hr/day max- $15/day; During the
summer, winter and spring breaks - all school aged children - $131.00/week;
$105.00/week part-time, $42.00/ full day.
A REGISTRATION
FEE of $25.00 is payable at the time of initial placement.
Registration is only valid for a two week period. If a child is withdrawn
and returns at a later date, an additional registration fee will be due at that
time.
A LATE FEE of
$25.00 will automatically be
charged when a payment is more than5 BUSINESS DAYS LATE. Another late fee of$25.00will be charged if late by 10 business
days. If no payment is received by the 15th business day your account will be
forwarded to our collection agency. If the collection agency does not
receive prompt payment this contract will be terminated.
A LATE PICK UP
FEE of $10.00 will be automatically charged for the first fifteen
minutes of lateness, with an additional $5.00 charged for each 5 minutes
thereafter. Payment is due at time of pick up.
RETURNED CHECK FEE of $25 will be charged for any returned checks.
SNOW DAYS : Please see our Greenway
Creative Learning Center Facebook Group for updates. Center may be closed
due to a Municipal Snow Emergency being declared in the city of Allentown.
SICK DAYS: In the event of EXTENDED ILLNESS: the full
fee is due for the first week of absence, half of the weekly fee is due for
2nd/3rd weeks of absence, after three weeks, absence fees will be waived
beginning with the 4th week. A registration fee will be due upon
return.
Please hand any prescription
medications to be given to your child to the teacher in charge, with
instructions to place it in the refrigerator or the closet. The parent
should then fill out the necessary information on the medication sheet.
The bottle must show the name of the child, prescription number, doctor and
dosage information.
Greenway will not
accept for care, not maintained in their care, children who have: Pink Eye,
Ringworm, Scabies, Lice , Croup, Diarrhea, Oral Temp of 100.0 or above,
Auxiliary temp of 99.0 or above, Rash with temp, Chicken Pox, Upper respiratory
infection with rash, Impetigo, Repeated or projectile vomiting, Viral Hepatitis
HOLIDAYS: The center will be closed for full days on
Thanksgiving Day, Christmas Day, New Year’s Day, Memorial Day, Independence Day
and Labor Day. The center will be closed for half days on Christmas Eve,
Good Friday, July 3.
SERVICE TERMINATION: Two weeks written
notice is required for TERMINATION OF SERVICE. If two weeks written
notice is not given, client will be charged for those two
weeks of service. Greenway-Parent Agreement Contract may be terminated at the
provider's discretion if it is felt that continued care of a particular child
might be detrimental to the child or other program participants. Greenway
has the right to cancel a child's enrollment or any other flagrant violation of
the policies of the program. Behavior will be dealt with on an individual
basis at the discretion of the administration.

this agreement I acknowledge that I have read and understand the Greenway Creative Learning
Center's Policy Contract.

permission to GREENWAY Creative Learning Center, LLC to use his/her likeness
for advertising programs at local supermarkets, our website and local
newspapers. I was not promised any compensation or payment for this permission
and will not seek same.
